Study Suggests Link Between
Sleep Deprivation, Alzheimer's Risk
Body seems to use brain's
downtime to clear away disease-related
material, researchers say
HealthDay
News -- A new study shows that
levels of amyloid beta, a byproduct of brain
activity that is considered a marker for
Alzheimer's disease, normally rise during
the day and decrease at night.
While the finding is preliminary, it could
suggest a possible link between sleep
deprivation and people's risk for developing
the brain-robbing disease, researchers say.
"We've known for some time that significant
sleep deprivation has negative effects on
cognitive [brain] function comparable to
that of alcohol intoxication," Dr. Stephen
Duntley, professor of neurology and director
of Washington University's Sleep Medicine
Center, said in a university news release.
"But it's recently become apparent that
prolonged sleep disruption and deprivation
can actually play an important role in
pathological processes that underlie
diseases. This connection to Alzheimer's
disease isn't confirmed yet in humans, but
it could be very important."
The findings were released online in advance
of publication in an upcoming print issue of
Archives of Neurology.
FAMILIES FACING
ALZHEIMER'S CAN EXPLORE MANY OPTIONS
DEAR ABBY: My mother-in-law was
diagnosed with dementia more than a year ago. She lives
alone and can no longer do anything for herself. She can no
longer climb stairs, which means she can't get to her bed,
shower or do laundry, and it takes her hours to dress
herself. She also needs help getting and remembering to take
her medications.
Because Mom can't drive, she can't get to the store, the
bank or anywhere else unless one of her kids takes her. She
does nothing all day but watch TV and eat sweets.
When do her kids stop treating her like a toddler and
start treating her with dignity? She clearly needs
assistance, whether it's a couple of days a week of
companionship or an assisted-living center. She doesn't want
to go, but when is it time to do what's best for her and
stop listening to what she says she wants? Her kids are
afraid to make her mad, so this poor woman is withering away
in her two-story home -- lonely, smelly, sad and depressed.
I would move her into our home if we had a place for her,
but we don't. What can be done for an elderly person who
obviously can't take care of herself, but "fakes" it so her
kids won't put her "in a home"? -- HEARTBROKEN
DAUGHTER-IN-LAW
DEAR HEARTBROKEN: Please accept my sympathy. Your letter
is timely, because September marks the inauguration of World
Alzheimer's Month.
Accepting the realities of dementia or Alzheimer's
disease can be difficult for families, especially when the
person with the disease has lost the ability to safely live
alone or make sound decisions.
When this happens, families must take steps to ensure
that their loved one is safe and healthy. This may involve
bringing care into the home or exploring other living
options. Fortunately, most communities offer resources that
can help, including home-delivered meal programs, in-home
care, assisted-living and residential memory care.
Because you are rightfully concerned about your
mother-in-law's well-being, call a family meeting and
discuss care options. The Alzheimer's Association has a
consultation program that helps families navigate through
these complex situations. It offers emotional support, needs
assessment and information about local resources. To speak
with a care consultant, call the Alzheimer's Association's
toll-free, 24/7 helpline at 800-272-3900.

When a patient shows up for a
doctor’s appointment with a companion, I often find
myself studying (and later, interacting with) them as much
as I do the patient. Most times, a quick review of body
language is all I need to make an assessment – is this
person looking out for the interests of the patient? Are
they here begrudgingly, or out of concern? Are they
indifferent, or attentive?
The daughters of two patients I
recently saw as outpatients illustrate the companion
archetypes I hold in my mind. One fell asleep while playing
with the cell phone in her lap as we explained to her mother
that she may have a life-threatening condition requiring
immediate treatment and hospitalization. The other walked in
with a pen and a legal pad, and proceeded to record
everything discussed about her mother’s health (she even
recorded her vital signs that morning) – at a routine
follow-up appointment.
It’s typical to see elderly
patients accompanied by their children when they can’t be
relied upon to understand and retain instructions delivered
by their physicians, but I’m also a fan of younger, more
independent patients bringing a companion or advocate. We
could all benefit from an extra pair of ears in the room, as
well as someone who may ask questions or raise concerns we
haven’t ourselves thought of. Additionally, it helps to have
someone who’s able to hold you accountable for your health –
are you doing what your doctor recommended?
In terms of compliance, we
speak of perceived severity and self-efficacy as proxies for
patients’ likelihood of adhering to their plan of care.
However, crucial to (and perhaps more important than)
achieving these conditions is an understanding on the part
of the patient. In my opinion, there is no better way to
realize this than to have a companion – a family member,
caretaker, or a friend – present at each doctor’s visit.
It has been estimated that
between 30-60% of patients 60 an older are non-adherent
to their medication prescriptions, and the incidence of
non-compliance increases with the number of medications
prescribed. For the elderly in America,
polypharmacy is the rule, not the exception, with
studies citing averages of 7 or 8 medications. When
compounded with declining cognition and independence
(sometimes a result of medications themselves), as well as
the level of health literacy most people have to begin with,
it’s a wonder we achieve a measurable level of compliance at
all.
Research has also shown that
Medicare beneficiaries who bring a companion are more
satisfied with their health care providers than
unaccompanied patients. For this and the reasons above, it
would make me happy to see more of an effort on the part of
public health organizations and individual physicians to
campaign for companions to be present at all visits to the
doctor’s office – especially for, but not restricted to, the
elderly.

Hurricane Preparedness
Excerpted from the Florida
Department of Elder Affairs
Disaster Preparedness Guide for Elders
Whatever your circumstances, each family or
household needs to have a plan to protect
itself during a disaster. This plan should
include what you will do and where you will
go in case of evacuation. As part of your
plan, organize a disaster preparedness kit
to sustain you and your family for seven
days. Most importantly, discuss your plan
with your family, friends, and neighbors.
Keeping your loved ones informed allows for
consistency and open communication should an
emergency arise. Should a disaster occur,
the plan you have in place will help protect
you from further distress.
Many seniors have specific medical needs
that must be considered. Make sure to have a
list of all of your medications, a generous
supply to hold you over, and a plan for
refilling prescriptions as needed.
According to Florida law, individuals with
special needs are eligible for evacuation
assistance should they need it. If you meet
those needs, and family or friends are not
available to help you, you may qualify for
assistance services. Register with your
local emergency management agency before
disaster strikes.

Caregiving
Did you
know that nearly 1 of 4 households in America provides some
type of care to friends or relatives over the age of 50?
Serving as a caregiver for a loved one can be very
stressful. Here are a few things you can do to help reduce
stress as you care for someone else:
Get support. Talk to a friend, hold regular
family meetings, or join a support group. Talking about the
problems you face as a caregiver can be very helpful.
Take care of your body. Make sure you are getting
enough sleep, exercise, and nourishing food.
Take a break. Find time for activities that lower
your stress level throughout the day.

faiths." Jewish Pavilion volunteers visit residents in 45
independent, assisted living and skilled nursing facilities

Care Association of Florida Elects Jim Mark as President
Tampa, FL
– June 11, 2010 –
The Private Care Association of Florida has elected Jim Mark
of American Home Companions as President of the association
at its annual conference.
Mr. Mark
is the President of American Home Companions. Based in
Orlando, American Home Companions has grown to become the
largest Live-in Companion Agency in Florida serving 21

industry, Mr. Mark has promoted affordable, non-medical,
in-home care options for Florida’s seniors.
Since
1977, the Private Care Association has been committed to
promoting the consumer directed model of care.
Consumer-directed care is reinforced by a philosophical
orientation that emphasizes the rights, within their
abilities, of the elderly and people with disabilities to
assess their own needs and make choices about what services
would best meet those needs. The principal advantages of
consumer-directed care are that it costs less, allows
consumers to individually select caregivers, provides
greater continuity in caregiver relationships, more
generously compensates the caregiver, and supports caregiver
entrepreneurship
